Club Dive Log and Risk Assessment - Form 307 (Dated 4/9/2006)

All dives to be logged and reported to the club Diving Officer (divers, groups, depths, times, weather, tides, dive site etc). CFT has issued a new dive log sheet/risk assessment which must now be used.
Please download the following form (in MS word if possible), Fill it in and Email it to diveofficer@vikingsubaqua.com .
